Rebate Program Details:

• Account must be open at the time we process the ATM Rebate.
• A separate form must be submitted for each account.
• The offer is good on ATMs in the United States only.
• ATM receipts must be received 90 days from the date the transaction was made.
• Forms must be mailed to the address above.
• ATM receipts must be dated January 1st 2006 or after.
